The Service Coordinator plays an important role in the overall coordination of services for children in the Early Intervention Program.
Duties & Responsibilities:
Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP
Ensure the IFSP outcomes and strategies reflect the family's priorities, concerns, and resources;and that changes are made as the family's/child's needs change
Coordinate the provision of all services that the child and family need or are receiving
Facilitate the timely delivery of services
Continuously seek appropriate services and situations that benefit the development of the eligible child for the duration of the child's eligibility
Perform Home Visits and attend IFSP meetings
Minimum 1 year experience as an EI Service Coordinator - Required
Must have excellent communication and organizational skills
Excellent per unit compensation
Combination of work from home / office (depending on your experience)
